We provide individualised service to our clients and their families to ensure that we deliver high quality and high standards every time. We encourage our clients to exercise greater choice and control over their care … and their lives.

Start with a Referral

Generally from an occupational therapist, physiotherapist, neuropsychologist, etc. – or from existing clients

A Conversation with the Client’s Referee

Get a good picture of the client’s situation, goals, and issues. If the potential client is looking to replace or supplement an existing agency, we look to understand what needs to be different to ensure an easy transition and successful care relationship.

A Conversation with the Client

If it appears that we will be a good fit for the client, we contact them directly to introduce ourselves and hear straight from them how we can meet their needs. We describe our usual approach to finding the right support workers from among our staff.

Find the Right Match

If the client is satisfied with what they have heard, we work behind the scenes to identify two (or more, as necessary) support workers who have the right combination of skills, experience, personality and availability to support the client in the way they have outlined.

Meet & Greet

Give the client the opportunity to meet personally with the support staff we have identified. If the chemistry is good, great; if not, we’ll try again until the right fit is found.

Documentation

Set out the forms that provide the boundaries of the relationship – making sure all parties are clear on each other’s expectations. All the forms are reviewed with the client (or representative) to be sure no questions go unanswered. Provide signed copies of all forms.

Multiple Contacts with Us

Ensure that the client has multiple contact points with Supreme: our support workers (for specific service delivery requests), our service coordinator (for issues related to the rostering of support workers), and our manager (for any issues related to the service delivery relationship).

Invoicing

Sent out regularly (usually fortnightly) either directly to the client or the plan manager.

Feedback

Requested of clients regularly, and at least annually on a formal basis, to correct any issues. We look to our clients’ families, their therapy teams, and funding agencies and plan managers to provide additional input on annual service delivery and support plans.

CHRIS
Manager

I believe in providing personalised care and strategies to help clients live their best and most independent lives. At home I’m passionate about the environment and protecting wildlife while enjoying my life, which is managed by my menagerie of dogs and birds.

Chris began in the disability field as a Support Worker in 2004, working up through roles as Head Support Worker, Program Coordinator and Senior Program Coordinator to become Supreme Health’s Manager in 2018.

Her extensive direct care experience, particularly in the Acquired Brain Injury and Spinal Cord Injury fields, combines with her Certificates in Disability and Aged Care and Diplomas in Business Management and Leadership and Management to work closely with her clients with disabilities, their families, support workers, therapy teams and funding bodies such as NDIS and TAC.

PHILIPPA
Service Coordinator

I’m passionate about helping people identify and achieve their goals, providing them with the resources and services required for a complete, fulfilling and safe life. In my free time, I love escaping with my husband to camp on our bush block and enjoy the wildlife.

Philippa has been providing support to people with disabilities since 2009. She has hands-on knowledge and skills supporting people with Spinal Cord and Acquired Brain Injuries in all activities of daily living. In 2017 she completed her Diploma in Community Services and advanced into her current role of Service Coordinator.

CAROLYN
Administrative Manager

Behind the scenes I can generally be found wrangling two cats and a husband; working on my endless dream to have my children’s books published; and getting my hands dirty volunteering for TreeProject and my local BushCare group.

Carolyn has worked in the disability field since 2007 in customer service and administration roles, mainly with people with Acquired Brain Injury and their families. She utilises her education and background to update Supreme Health’s policies and documents, and provides administration support for the team and people with disabilities.

Owners

“We feel incredibly privileged to step into a business that reflects our values and our belief in doing things properly - with honesty, reliability and genuine care. It’s an honour to support a team so dedicated to making a real difference in people’s lives. When we’re not working, you’ll usually find us out with our dog, who keeps us active and outdoors.

John and Lindy became the owners in 2026 with the aim of building on the business’s long history as a supportive, family‑style provider for people with disabilities. John brings experience in general operations, IT, and analytics, while Lindy contributes her background in accounting, finance, and management consulting. Together, they support the care team by providing strong operational and financial oversight behind the scenes, ensuring staff can focus on what they do best – supporting clients with consistency and care.
